a. Authorization signed by the patent owner. Name, address, nationality and tax identification number of the applicant (legal or natural person). In the case of a legal person, it is necessary to indicate the name and position of the person signing. (In case of a natural person, the name should be the same that appears on the National Identity Card; for legal persons, the name must reflect the particulars recorded in the Commercial Register). b. Declaration of inventorship including name, address and nationality of the inventor/s, as well as indication of the working relationship between the inventors and the applicant company or entity (for instance: employment contract, private contract, etc.). This document is to be signed as well by the patent owner. In the case of a legal person, it is necessary to indicate the name and position of the person signing.
